All participants must sign the official MSF Safety Course General Release, Waiver & Indemnification Agreement before riding. Download and print it below.
Riders under 18 — important
Per the MSF waiver: a parent or guardian must sign IN PERSON at the training location, or the form must be NOTARIZED and brought to class. Without a properly completed waiver, minors will not be permitted to ride.

What to do
- Download and print the waiver above.
- Complete the participant information fields.
- If you are 18 or older: sign both sections and bring the completed form to class on Day 1.
- If you are under 18: a parent or legal guardian must sign IN PERSON at the training location on Day 1, or the form must be NOTARIZED beforehand and the notarized copy brought to class.
